Grace City Cares Home Care Agency

1. Introduction

These Terms of Services (“Terms”) govern the provision of home care services by Grace City Cares Home Care Agency (“Agency,” “we,” “our,” or “us”) to clients (“Client,” “you,” or “your”). By engaging our services, you agree to these Terms.

2. Services Provided

The Agency provides non-medical in-home care services, including but not limited to:

  • Personal care (bathing, grooming, mobility assistance)

  • Companionship and supervision

  • Meal preparation and light housekeeping

  • Transportation and errands

  • Medication reminders

We do not provide medical, nursing, or clinical services unless otherwise specified in writing.

3. Care Plans

Services will be based on a customized care plan developed in consultation with the Client and/or family members. Care plans may be adjusted as the Client’s needs change.

4. Scheduling and Hours of Service

  • Clients may request regular or flexible schedules based on availability.

  • A minimum of [1-2 Hours] per visit may apply.

  • Changes to schedules must be requested at least 24 hours in advance.

5. Fees and Payment

  • Service rates will be provided in writing before care begins.

  • Payment is due [weekly/bi-weekly/monthly] via check, electronic transfer, or other approved methods.

  • Late payments may incur additional fees.

6. Cancellation Policy

  • A minimum of 24-hour notice is required to cancel or reschedule a visit.

  • Cancellations made with less than 24-hour notice may be subject to a cancellation fee.

7. Caregiver Assignment

  • The Agency will assign caregivers based on availability, qualifications, and Client needs.

  • While we strive to maintain consistency, we cannot guarantee that the same caregiver will always be available.

8. Client Responsibilities

The Client (or their representative) agrees to:

  • Provide a safe home environment for caregivers.

  • Treat caregivers with dignity and respect.

  • Inform the Agency promptly of any changes in health condition, schedule, or care needs.

9. Confidentiality

We respect and protect the privacy of all Clients. All personal and health-related information will remain confidential in accordance with applicable privacy laws.

10. Termination of Services

Either party may terminate services with [insert notice period, e.g., 7 days] written notice. The Agency reserves the right to terminate services immediately if the caregiver’s safety is at risk or if the Client violates these Terms.

11. Limitation of Liability

The Agency is not liable for medical, financial, or personal decisions made by the Client. We are not responsible for injuries or damages beyond the scope of the services provided.

12. Governing Law

These Terms shall be governed by and interpreted under the laws of the State of Arizona.
